DVA Circular – Pooled Swab Testing

Dear Residents,

We would like to inform you that Dasmarinas Village has an opportunity to participate in the Pooled Swab Testing Pilot Program being conducted by the City of Makati. Our security and maintenance personnel will be participating in this program and want to extend this opportunity to household staff working inside our village.

We are surveying to find out the number of people interested in participating. The information will then be sent to the Office of the Mayor.

  1. Participation in this Pooled Swab Testing Pilot Program is free, courtesy of the City of Makati.
  2. Testing will be done at the Makati Coliseum and transportation will be provided from Dasmarinas Village and back. No date for the testing has been set.
  3. This program is attempting to detect asymptomatic patients. If an individual is showing symptoms of COVID 19 or is suspected of being COVID-19 positive, please do not register for this program.

a. Full Name
b. Street Name
c. E-mail Address
d. Mobile Phone Number
e. Number of your household staff that wants to participate

You will receive an e-mail confirmation after you have registered.

Please remember that this link is only for a survey to establish the number of Dasmarinas Village participants. Once the plan is finalized, participants will be asked to provide more information via an online registration system.

We have been given until Friday, September 11, 2020, to provide the number of participants from Dasmarinas Village. Please follow the link and fill up the simple survey questionnaire AS SOON AS POSSIBLE. We will close the survey by noon on Friday and send out the information soon after that.

Remember, the link is for the survey to establish the number of participants from Dasmarinas Village. It is NOT the online registration for the program.
