CSA Stickers Application

NOTE: This page is for the CSA (Colegio San Agustin) Vehicle Stickers.

Thank you for applying for a DVA-CSA Vehicle Sticker. Please read through the steps below to help you understand the process for applying for a DVA-CSA Vehicle Sticker.

Note for those who have previously applied online:

  • If you previously applied online but have not received any confirmation email, please bring copies of the required documents.
  • If you previously applied online and have already received an SOA but not yet paid, please bring a copy of your SOA and pay the corresponding amount.
  • If you previously applied online and have already paid, please bring your confirmation email.

Step 1: Apply

  • Please download the CSA Sticker Application Form and fill it out.
  • Please bring the filled out form, together with all applicable requirements listed below.
    • We will have a booth in front of Urdaneta Hall.
  • Please bring the vehicle you are applying for.
  • Please bring all requirements:
  • Application Requirements:
    • Vehicle Certificate of Registration (CR) and latest Office Receipt (OR);
    • If vehicle CR or OR is not yet available, Sales Invoice and Delivery Receipt.
    • For vehicles not under the name of the applicant, a certificate from the vehicle owner designating the vehicle to the sticker applicant.
    Additional Requirements:
    • For Parent(s)/Legal Guardian(s): CSA Official Receipt of Student Registration
    • For CSA Teachers/Staff: Valid CSA Teacher/Staff ID
    • For School Service Operators: Valid Accreditation from CSA
  • Applications are from 8am to 12 noon.

Step 2: Payment

Once your application has gone through our verification process and has been approved:

  • You may pay immediately at our booth in CSA.

Step 3: Installation

Once payment has been verified:

  • Attachment of stickers will be at our booth in CSA.
  • Please bring the vehicle you are applying for.
