BAKUNA MAKATI IN BARANGAY DASMARIÑAS VILLAGE

Dear Barangay Dasmariñas Community,

The Makati City BAKUNA MAKATI COVID-19 vaccination drive is coming to Dasmariñas Village.

The Barangay Dasmariñas Vaccination Site will be ready to administer BAKUNA MAKATI vaccinations to SENIOR CITIZENS STARTING ON APRIL 14, WEDNESDAY AT THE COLEGIO DE SAN AGUSTIN AIRCONDITIONED GYMNASIUM. Please expect text/SMS messages from BAKUNAMKT with the date and time of your vaccination appointments.

Please observe the following guidelines:

1. Only those who received text/SMS messages from BAKUNAMKT will be entertained. No walk ins allowed. Companions will be allowed only if it is clear upon registration that they are necessary.
2. Please observe the assigned schedule and be there on time. Please do not come too early to avoid overcrowding.
3. Please bring the following:
   a. Face mask and face shield.
   b. The actual device that received the text/SMS message from BAKUNAMKT. You may be asked to display the actual text/SMS message showing your vaccination schedule.
   c. The confirmation code that you received when you registered for the vaccine. This can be printed or as a screenshot.
   d. A government-issued identification card.
   e. Your own ballpen.
   f. If you have comorbidities, medical documents with information on your comorbidities (e.g. medical certificate, prescriptions, etc.).
   g. This is optional – A smartphone or tablet. The vaccination process includes an e-signature and a survey that are done online. If you do not have a personal device, there will be devices on hand for public use.
